One of the most common questions small therapy practice owners ask is: "Do I need a full billing system just to collect copays from my clients?" The answer, for many practices, is no.

If your billing workflow is straightforward — you collect copays at time of service, you bill insurance through a clearinghouse or billing service, and you occasionally need to collect a self-pay fee or a balance — you don't need a $100 to $200 per month billing module bolted onto your practice management software. You need a simple, reliable way to charge a card.

Practice management platforms bundle billing with everything else — scheduling, clinical notes, intake forms, telehealth — and price the whole package per provider. A 5-provider practice paying $59/provider/month is spending $295/month, and the billing module might be the only reason they're on the platform at all. Everything else could be handled by simpler, cheaper tools.

What Payment Collection Actually Looks Like in a Small Practice

Most small therapy practices have a few payment scenarios that repeat:

Copay collection. Insurance client comes for a session. They owe a $25 to $75 copay. You collect it before or after the session.

Self-pay session fees. Client without insurance (or out-of-network) pays the full session rate — typically $100 to $250 per session.

Sliding scale fees. Reduced rate for clients who qualify. Could be any amount based on your sliding scale structure.

Evaluation deposits. Some practices collect a deposit for initial evaluations, applied to the balance after insurance processes.

Outstanding balances. After insurance pays its portion, the client may owe a remaining balance. You need to collect it.

Group or workshop fees. If you run therapy groups, parenting workshops, or psychoeducational classes, participants pay a flat fee.

None of these scenarios require a full revenue cycle management system. They require the ability to charge a card.

Three Ways to Collect Without a Billing System

1. Standalone Payment Links

Create a payment link with the amount, a description (e.g., "Session copay — March 15"), and the client's email. Send it by text or email. The client clicks, enters their card, and pays. You get a receipt. They get a receipt. The transaction is logged in your payment dashboard.

For recurring copays, you can create a payment link template and reuse it — just update the amount and description for each session.

2. Form-Embedded Payments

If you're already sending clients a form — an intake questionnaire, a consent update, a satisfaction survey — embed a payment field directly in the form. The client completes the form and pays in the same flow. This is particularly useful for evaluation deposits: the parent fills out the intake packet and pays the deposit in one step.

3. Text-to-Pay

Send a payment link directly via text message after a session. The client taps the link, pays on their phone, and walks out. No card terminal. No awkward "the card reader isn't working" moments. No waiting for a slow chip reader.

What About Card-on-File?

Many practices keep a card on file and charge it after each session. This works but creates PCI compliance obligations — you're storing card data, which means you need to handle it securely.

Payment links sidestep this entirely. You don't store the card. The payment processor (Stripe) handles the card data. You just send a link and get paid. If the client wants to pay with a different card next time, they can. No updating stored information.

If your practice collects $10,000/month in copays and self-pay fees, your processing fees are roughly $320/month regardless of which system you use — the card networks charge the same rates. When You Actually Need a Billing System

To be fair, there are scenarios where a full billing system is worth the cost:

  • You bill insurance directly (not through a billing service) and need claim submission, ERA processing, and denial management
  • You have complex billing scenarios with multiple payer types, prior authorizations, and modifier codes
  • You need automated superbill generation integrated with your clinical notes

If those describe your practice, keep your billing system for billing. But use a separate, flat-rate operations platform for intake forms, consent signatures, records management, and simple payment collection. You'll still save money by not needing every team member on the billing platform.
